Registration Guidelines:

Team Player Registration:

Team registration and rostering is open only to players whom have been selected after an official tryout and/or selected by the team coach. Upon selection, the team manager will have the player and parent fill out registration forms and submit a copy to the Club Administrator. The following registration forms must be submitted:

1. USYSA League Player Pass or Player Status Form – if applicable.
2. USYSA Region I Player Pass – if applicable
3. US Club Soccer Player Registration & Players Pass – if applicable.
4. Player Medical Release Form.
5. A passport size player photo – Must be taken in TAP Uniform.
6. TAP Futbol Club MEMBERSHIP Registration Form (MANDATORY).
7. Signed Parent Code of Conduct.
8. Signed Player Code of Conduct.


DOCUMENTS AND FORMS

NOTE: Each player will be required to provide an original copy of their birth certificate (other proof of birth date is acceptable – based on USYSA and/or US Club Soccer requirements)
